A lab may believe it is ready for an FDA inspection because its SOPs are current and the quality manual is polished. Then the inspector asks who approved a particular change, which version of an SOP governed the work on a specific date, or where the raw validation data can be found for a specific validation run. The laboratory can clearly perform the work, but if it cannot reconstruct the record quickly enough to answer straightforward questions, the inspector’s questions are likely to become more challenging.

Missing signatures and outdated policies matter. More significant findings usually emerge when the laboratory cannot explain how its quality system functions. These findings can trigger remediation projects that delay product timelines, complicate financing discussions and create additional diligence requests during transactions. Sponsors and investors treat inspection outcomes as credibility signals, and a weak inspection story can slow fundraising or inject new representations and warranties into a deal.
